Form T2201

Before you print the T2201, check its version

Since September 8, 2026, the Canada Revenue Agency has refused any Form T2201 dated before 2023. A copy printed last year, or a blank copy kept on file at a clinic, can be refused even when every box is filled in correctly.

How to tell whether your copy is the right one

Every version of the form carries the year it was revised. If yours is older than 2023, or you cannot find the year, download the current version rather than risk a refusal.

Download it from the Canada Revenue Agency’s form page, then print it. We keep no copy of the form here: a copy hosted anywhere else can go out of date without anyone noticing, which is exactly what gets an application refused.

Or print nothing at all

You can also apply online, in your CRA account: the online form is always the current version. Do not use the “submit documents” section for a new application. Since July 14, 2026 it has not accepted them, unless the CRA has asked you for more information.

Who fills in which part

The form has two parts. You fill in Part A. A health professional fills in Part B, and it is not always a doctor: depending on the restriction, it can be, for example, an optometrist, an audiologist or an occupational therapist.
